Job summary
Mission Hospice & Home Care serves patients and families in the San Francisco Peninsula and South Bay with quality care and compassionate end-of-life support and education.
+- Provide nursing care to patients who require GIP level of care for end-of-life symptom management and pain control in collaboration with the Stanford Hospital inpatient team.
Responsibilities
- Perform initial and ongoing hospice eligibility assessment and consultation with Hospice MD in collaboration with Stanford team and in accordance with CMS guidelines
- Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions
