Director of Social Services Position Overview:

As the Director of Social Services, you will oversee the social work staff in assisting residents with social, emotional, and related fields associated with the medical condition and long-term care goals.

Director of Social ServicesKey Duties:

Ensure the resident(s) have a smooth transition into the facility.
Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (physical, cognitive, and socialization factors) in planning social services programs.
Knowledge, skills, and techniques necessary to plan social services programs with the following needs: therapeutic, neurological and dementia, dialysis, IV therapy, and infectious diseases if applicable.
Maintain progress notes under federal and state guidelines for Long Term Care.
Promptly follows-up on admission inquires, evaluates, and assesses the needs of prospective residents to determine appropriateness of facility placement.

Director of Social Services Qualifications:

Bachelor's degree required.
CSW or LSW required.
Two plus years' experience with Seniors. Skilled long-term care preferred.
Knowledge of Local, State and Federal regulations and the survey process.
