- Responsible for planning, development, organization, implementation and ongoing operations of the hospital care management department
- Provides leadership and direction to care coordinator staff members
- Ensures the coordination of services for a smooth continuum of care process throughout the organization
- Develops and implements standards of operation, policies & procedures, work flows and desktops to ensure quality management, fiscal accountability and outcomes measures
- Ensures compliance with regulatory agencies
- Unencumbered RN license to practice in the State of Arkansas, or compact state
- Bachelors Degree required, preferably BSN
- Minimum of 5 years nursing leadership experience, preferably in an acute care setting
- Minimum of 2 years in a case management setting, preferably in an acute care setting
- CCM Certification preferred

As our Director of Care Management,now part of CommonSpirit Health formed between Catholic Health Initiatives (CHI) and Dignity Health, youll be part of a leadership team that ensures our patient care experience is the best it can be As a leader in care management, you will lead a team that works with physicians, other nursing teams, specialized departments, and various outside agencies to ensure that our patients are taken care of; well after they are discharged from our care.
Youll build trust in these interactions by showing compassion, using your critical thinking skills, and remembering that each patient requires a custom care plan tailored to their needs. You will lead a team of professional care managers that support patients by identifying and removing patient care barriers that will regress their healing.
