    Our Team


    Our Extended Care team consists of board certified physicians, nurse practitioners, and physician assistants who provide episodic and chronic care to patients staying in several skilled nursing or long-term care facilities across the North Shore.

    Schedule

    This is a full-time, 40 hour position. The desired schedule is Monday through Friday from 8:00am to 5:00pm, with night, weekend, and holiday on-call rotation.

    We do our best to publish the on-call schedule in advance for our NPs to plan accordingly and/or swap shifts if necessary.

    Locations

    Potential skilled nursing or long-term care facility assignments include Beverly, Salem, Peabody, Lynn, and/or Marblehead. Employees are ultimately assigned to one home location, depending on candidate skills/competencies and departmental need. This will be determined during the interview process.

    General Summary


    The NP assists individuals and families with their care needs by obtaining health and medical histories, performing physical examinations and assessing health problems.

    The NP cares for patients with acute and chronic life-limiting illnesses by managing therapeutic regimens, prescribing medications and performing other studies and activities that are deemed appropriate.

    New and follow-up patients will be seen and evaluated by the NP, with physician input obtained as necessary and as outlined in the Nurse Practitioner Practice Guidelines.

    The NP will assume SNF/LTC care management responsibilities and will facilitate communication with the patient, the patient's family or significant other, and the referring physician.

    The NP will work in collaboration with the patients' attending physicians at the SNF/LTC facility, as well as provide continued collaboration with the patients' primary care provider(s).

    Required


    • Master's of Science with certification in Adult, Family, or Geriatric Nurse Practitioner
    • Active Massachusetts APRN licensure
    • Controlled substance licenses (Federal and State)
    • CPR certification
    Experience with adult or geriatric care strongly preferred
