This position is based in Ft Myers, FL, serving patients in Lee County
Your Role as a Licensed Practical Nurse (LPN):
- Deliver high-quality nursing care to patients in their homes based on individualized care plans.
- Perform a variety of tasks, including:
- Acute and chronic disease management.
- Medication management and administration.
- Assessing patient conditions and reporting changes.
- Developing tailored, goal-oriented treatment plans.
- Post-surgical rehabilitation.
- Educating patients and caregivers to support recovery and independence.
- Pain management and wound care.
Qualifications:
- Unencumbered Florida LPN License (required).
Valid driver's license, auto insurance, and reliable transportation (required).
IV Certification (required).
- Experience in acute care or hospital settings (preferred).
- Strong assessment and care planning skills.
