- Bachelor's degree in a clinical behavioral health field (i.e., counseling, social work), preferred
- Two+ years of experience in case management, care coordination and/or behavioral health
- Potential for competency in multiple Electronic Heath Record Systems
- Ability to manage one's own time efficiently
- Bilingual in Spanish is preferred, not required

Care Coordinator in Phoenix
The Care Coordinator is a key member of the Behavioral Health Team and serves as an advocate for patient by coordinating care to minimize the wait for services, effectively assess needs of patients, and increase engagement in therapy and integrated services.
Location will be hybrid work-from-home as well as on-site at one or more primary care practices in the Phoenix, AZ area.
Compensation
