    Job Summary

    The Service Coordinator provides basic case management and daily coordination of supportive services for residents, with a focus on frail and at-risk individuals. Services may include personal assistance, homemaking, meals-on-wheels, transportation, health screenings, and advocacy.

    Essential Job Duties and Responsibilities

    • Establish relationships with local agencies, service providers, and community resources to arrange individualized and flexible services.
    • Maintain a directory of community-based providers for staff and resident use, ensuring timely referrals and regular updates.
    • Monitor lease compliance and inform the Community Manager of any noted violations.
    • Identify at-risk residents through observation, referrals from management, outside sources, or personal assessment.
    • Perform basic case management, including:
      • Assessing individual needs for service based on Activities of Daily Living (ADLs)
      • Arranging appropriate services as liaison between residents and care givers
      • Monitoring service quality and ongoing reassessment of resident needs
      • Follow-up and disposition/termination of services
    • Maintain secure, locked files on each resident client, documenting case management steps, reports, progress notes, and follow-up.
    • Brought wellness and preventative medicine screenings, clinics, and speakers on medical and legal issues into the property.
    • Foster informal support networks among residents, family, and friends.
    • Work with the Community Manager to assist residents in accessing necessary services for continued residency or higher-level care.
    • Submit monthly reports on program activities to the Coordinator for Service Programs and the Community Manager.

    Education and Experience

    A Bachelor's Degree in social work, gerontology, or other social science is preferred, but five years of relevant experience may be considered in lieu of a degree. Evidence of networking ability and senior citizen relations skills are required, along with experience arranging services for the elderly.

    Line of Authority

    The Service Coordinator receives supervision from the Community Manager on administrative matters and from Regional Service Coordinators on programmatic issues. In the Community Manager's absence, supervision will be received from the Regional Manager.
