    I. JOB SUMMARY:

    Responsible for clinical oversight of the Rosa Coplon Long Term Home Health Care Program and Menorah Licensed Home Care Program

    II. PRIMARY JOB DUTIES:

    • Manages and directs home care services provided by the programs
    • Provides clinical oversight to both the LTHHCP and MLHC Program.
    • Assumes the responsibilities of the Director of Community Service during their absence
    • Plans, organizes, directs, coordinates, and evaluates the LTHHCP and MLHC Program
    • Recruits and hires qualified professional staff for the program.
    • Completes performance evaluations of staff assigned.
    • Plans and participates in orientation and in-service programs.
    • Assists in the preparation and administration of the program budget.
    • Determines the staffing pattern to accomplish program objectives.
    • Serves as a resource and consultant to staff.
    • Participates in development, review, and revision of policies and procedures of the Program.
    • Develops job descriptions for the Program as requested.
    • Coordinates the Program's services with the services provided by other community agencies.
    • Serves as chairperson of the Professional Advisory Committee.
    • Serves as the liaison to the community for the LTHHCP and MLHC.

    III. OTHER JOB DUTIES:

    • Knowledge of the Department of Health and CMS guidelines.
    • Knowledge and ability to apply community health principles and practices.
    • Ability to manager program personnel.
    • Ability to work independently within the framework of the team setting.
    • Ability to enlist the cooperation of many people in furthering the program.

    IV. JOB SKILLS:

    • Represents agency on community boards and committees.
    • Facilitates establishing of goals and objectives.
    • Monitors service delivery through review of patient records to determine adequacy of documentation, utilizing services, plan of care outcomes.
    • Interprets agency policy and procedure to staff.
    • Initiates or participates in patient centered conferences to improve general patient care.

    V. JOB SPECIFICATIONS:

    Bachelor's Degree in Nursing from an accredited college or university. Master's Degree in Nursing or a field relevant to Community Health Nursing Administration is required if more than 30 full-time or equivalent professional staff positions are budgeted.

    License/Certification:

    License and current registration to practice as a Registered Professional Nurse in New York State.
