Service Coordinator Role in Amherst, MA - Amergis
Description
Amergis Healthcare Staffing is partnered with a much respected facility in Amherst MA who is currently seeking Peer Counselors to assist them at their Independent Living Facility.
Details of the position include:
Location: Amherst, MA
Duration: 13 weeks to start, but strong chance of extension
Facility Type: Independent Living Facility
Openings:
+ 8a-6p Mon-Fri
Start Date: Two weeks from offer
POSITION DESCRIPTION:
+ Conduct LTSS assessments; by identifying the needs and recommending services and resources
+ Connect consumer to social services and community resources
+ Participate on the consumers care team
+ Perform outreach
+ Advocate for the member
+ Provide LTSS care coordination
+ Initial and quarterly home visits
+ Remain current with community resources
QUALIFICATIONS:
+ Must have knowledge of community resources and services available for people with disabilities.
+ Excellent computer skills, knowledge of MS Office and database navigation
+ Excellent communication and writing skills to ensure respectful delivery of services
+ Excellent time management and organizational skills
+ Must have valid MA driver's license and reliable transportation as well as proof of insurance if applicable.
