    Summary



    The incumbent functions in a multidisciplinary outpatient Mental Health clinic setting within the Clement J. Zablocki VA Medical Center and Community Based Outpatient Clinics (CBOC's).



    Duties

    • Independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs.
    • Independently conducts psychosocial assessments.
    • Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team especially related to mental health concerns.
    • Able to coordinate community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs.
    • Identifies high-risk patients and provides case management services.
    • Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members.
    • Establishes and maintains effective therapeutic relationships with veterans and their families.
    • Provides consultation services to other staff regarding the psychosocial and mental health needs of veterans and the impact of psychosocial and mental health problems on health care and adherence to treatment plan.
    • General knowledge of veterans' benefits and services relating to special programs, service connected compensation, non-service connected pension.
    • Able to meet the needs of customers while supporting VA missions.
    • Relies on SUD clinical experience to guide initial and ongoing evaluation for veterans self-presenting or in response to consultation requests.
    • Makes recommendations for treatment or referral in cases where consequences for the veteran may have serious effects on current and future life circumstances.
    • Develops individualized treatment plans for assigned SUDS patient cases.
    • Initiates SUDS evidence based treatment, documenting patient progress, amending treatment plans as response indicates need and in accordance with Medical Center policy and/or accrediting body standards.
    • Provides clinical case management services to assigned SUDS caseload, using extensive SUD clinical experience to guide care integration to a course most probable to produce stable SUD recovery.
    • Explains client's treatment and progress towards rehabilitation to third parties.
    • Provides recovery-oriented continuing care treatment services to assigned cases with the goal of establishing the Veteran in the SUD recovery process.
    • Works effectively with patients who tend to have frequent and severe crises, lack family or an adequate community support network, be poor at self-monitoring, frequently fail to comply with instructions and treatment, or have significant deficits in coping skills and require continuing professional psychosocial support.

    Work Schedule: Monday-Friday 8:00 am - 4:30 pm

    Pay: Competitive salary and regular salary increases.
