- Completes psychiatric nursing and health care assessments and documents in the medical progress notes within 24 hours of service delivery.
- Participates in the development of treatment plans for clients that need psychiatric intervention, to include the formulation of short and long term goals, evaluation, and treatment plan modification for each person that receives nursing services.
- Completes a nursing progress note for each face to face service contact which is done with the individual, conforming to all DSHS Community Standards and Medicaid Standards.
- Monitors and records vital signs.
- All verbal orders, which are received from the physicians, are documented within 24 hours of receipt of those orders.
- Acts according to all written and verbal medical orders in accordance with Texas Statute.
- Records all telephone contacts accurately within 24 hours of contact in the Medical Progress notes.
- Gives and records injections according to medical orders and agency guidelines after each occurrence.
- Checks and records blood pressure of all identified hypertensive clients and Clozaril clients at each nursing appointment, and according to MHMR of Tarrant County Vital Signs parameters.
- Refers clients with severe medication reactions for emergency treatment according to the guidelines established by the agency.
- Performs and records an AIMS Examination Procedure on each client exhibiting symptoms suggestive of first time Tardive Dyskinesia.
- Shows evidence of health and/or medication teaching related to the Treatment Plan in the notes. Evaluates needs/outcomes of those receiving the instruction and documents each occurrence.
- Assumes accountability when using independent clinical judgment and established protocols. Exercises the option of team consensus gathering when faced with decisions that involve other disciplines or complex clinical issues.
- Assist the physician to ensure compliance with lab studies and the policies regarding the standard of practice for lab reviews.
- Always promotes collegiality among health care team members, through collaborative communication.
- Maintains stock of injectable medications and emergency medication supply for the clinic, stock will be checked at least every two weeks to reorder.
- Collaborates with other health care providers and members of the organization to solve clinical care problems each time they occur.
- Maintains necessary supplies for implementation of agency policies on infection.
- Always maintains confidentiality of all clients, observes preservation of client rights including the right to file a complaint.
- Communicates with Director/Assistant Director/Program Manager regarding complex clinical or ethical issues, each time they arise.
- Actively promotes wellness oriented lifestyle behaviors for those individuals we serve as evidenced by nursing progress notes.

I) Job Purpose
Under the supervision of the Director/ Program Manager, the responsibilities of this position are to provide quality nursing care to assigned clients and to be a member of the interdisciplinary treatment team.
II) Essential Functions
A. Ensure the delivery of quality client care through the nursing process by utilizing the resources available to provide initial and ongoing assessment for agency clients who require psychiatric treatment under the supervision of the physician and Registered Nurse.

This position requires driving to and from locations for work and/and may require overnight traveling.
