- Exhibit excellent communication skills, especially when working with seriously ill or terminal patients and their families.
- Collaborate closely with the medical director, team physicians, and leaders to maintain continuity of care aligned with hospice principles and patient preferences.
- Effectively work with all members of the hospice team, ensuring seamless coordination.
- When providing inpatient visits, share the responsibility for call duties as part of the rounding process.
- Ensure all documentation is completed within 24 hours of the visit.
- Maintain an active license and DEA registration in the state where care is delivered.
