    Resident Care Coordinator Job Description


    Purpose:


    To manage care giving staff; assist in coordinating the daily administrative and care giving functions of the Community under the direction of the Executive Director; assist the nursing staff with communication with residents, families, physicians and other outside healthcare providers; organization of supplies and materials; assist with the maintenance of resident records; and also supervise the personal care of residents and assist in monitoring the wellbeing and the environment of the residents.


    Minimum Eligibility Requirements:

    • CNA, or previous experience working with the elderly in a residential or long-term care setting preferred.
    • Current CNA license, other license, or ability to obtain as required by State regulatory agency.
    • Must enjoy working with seniors.
    • Proficiency in basic computer skills, including Word, Excel, and Outlook.
    • Must have a strong commitment to customer service and desire to exceed customer expectations.
    • Ability to establish effective relationships with residents, family members and team members.
    • Must be able to read, comprehend, write, and speak English to communicate effectively with residents, families, visitors, vendors, physicians and their office staff, and other team members.
    • Ability to represent the community in a positive and professional manner.
    • Possess strong interpersonal and communication skills, both written and verbal.
    • Consistently exercises good judgment in making decisions and recommendations.
    • Must be highly motivated, with strong organizational skills, attention to detail and accuracy.
    • Possess excellent time management skills with ability to work independently as well as collectively on assigned tasks.
    • Ability to handle multiple responsibilities effectively completing projects with minimal supervision and meeting deadlines with frequent interruptions in a fast-paced environment.
    • Ability to re-prioritize daily tasks in order to accommodate fluctuating needs of the community.
    • Ability to maintain confidential information, including but not limited to resident health, community, and team member information.
    • Must meet all health-related requirements pertaining to state licensure.
    • Must maintain current CPR/First Aid credentials.
    • Must maintain current Food Handlers credentials.
    • Must maintain annual Continuing Education Credits.
    • Must be able to perform duties and responsibilities (Essential Job Functions) with or without reasonable accommodation.

    Essential Functions:
    Management


    • Provide supervision for care team, including hiring, training, evaluation, and discipline.
    • Develop and post schedule and ensure adequate staffing on a daily basis. Communicate with care team about meetings, in services, etc.
    • Available for after-hours staffing concerns- Community will attempt to solve all staffing issues prior to calling RCC.
    • Coordinate care team orientations and trainings with the Business Office Manager/HR Manager
    • Teaches in service topics and orientation classes as directed by the Executive Director.
    • Serves as member of the management team and assists Executive Director with other management duties as needed.
    • Assist with team performance concerns and/or guidance through mentoring.
    • Complete care team performance evaluations.
    • Exercise independent judgment and discretion in the performance of job duties.
    • Responsible for maintaining resident records.
    • Prepare work orders for maintenance department.
    • Establish and maintain positive and professional working relationships with residents, and all team members.
    • Attend management meetings.
    • Maintains knowledge of current OSHA and state regulations, and routinely monitors Community compliance with the regulations related to resident care and nursing functions.
    Resident Care


    • Reviews and understands all resident individual service plans.
    • Directs resident care and activities as needed.
    • Observes residents for changes in physical, emotional, mental, and social condition, unusual symptoms, accidents, injuries, or unusual occurrences, and reports promptly to a licensed nurse or Resident Coordinator.
    • Completes all forms such as incident reports, and written and computerized documentation, as necessary for communication of resident information with other shifts, Resident Coordinators, and with the licensed nurses.
    • Ensures that changes in resident condition are monitored, documented, effective interventions are implemented, and reported appropriately to nurse's physician and/ or family members.
    • Provides support and assistance to medication management systems and ensures that medications are dispensed in a safe and effective mariner.
    • Ensures that weights and vitals are done monthly on all residents and records are maintained in resident chart/profile.
    • Acts as a role model to other team members in demonstration of courtesy, gentleness, effective approach, communication and behavior management strategies, and engaging residents in meaningful activity and interaction.
    • Ensures that all resident care is provided in a safe and effective manner, while consistently maintaining resident dignity, choice, and respect.
    • Assist in ensuring that resident service plans accurately reflect resident care needs and updated as needed.
    • Assists in leading family care conferences.
    • Serves as a liaison between resident, family, and physician regarding resident health care.
    • Coordinates and schedules resident transportation with family members, community vehicles, or outside transportation companies.
    Housekeeping


    • Manages care team for completion of housekeeping during shift.
    Other


    • Attends all required team member meetings and in-service training sessions.
    • Answers phones as needed in a prompt and courteous manner.
    • Upholds Residents' Rights as defined by state regulations and by Company policy, at all times.
    • Supports and upholds all other Company policies, and the service philosophy.
    • Reliable and predictable attendance.
    • Other duties as assigned.

    Working Conditions:

    • Must be able to work full-time, including occasional evenings and some weekend and holiday rotations.
