    Summary of Responsibilities:

    The Clinical Case Manager serves to provide clinical support throughout the assessment, planning, and treatment process. The Clinical Case Manager assists in identifying and formulating highly individualized, targeted treatment goals and monitors client progress with respect to therapeutic goals. The Clinical Case Manager coordinates with TTC admissions for completion of the client portal, intakes and follows the client through the program process to discharge. The Clinical Case Manager works closely with clinical staff, utilization management and alumni coordinators to identify ways in which clients require additional clinical support, case management needs, or documentation needs. Clinical case manager also supports with needed clinical coverage or relevant research to support client care.

    Essential Duties:
    • Provides ongoing clinical support to clients to include completion of client intake portal, clinical assessments, treatment planning, and supportive counseling for ongoing stressors and crises.
    • Administers specialty assessments that may supplement and support therapeutic goals including strength and interest inventories.
    • Monitors, evaluates, and records client progress with respect to therapeutic goals.
    • Provides support to interns with respect to orientation.
    • Completes research projects as directed by supervisor
    • Provides clinical coverage as directed by supervisor to include group or individual work
    • Understands the need for appropriate client/staff boundaries - discontinues communication with clients upon discharge other than through official TTC contact. Informs supervisor of any client contact after discharge.
    • Meets quality expectations of accuracy, documentation and record-keeping.
    • Exhibits excellent customer relation skills as evidenced by supportive and constructive communication with all contacts including coworkers, clients, visitors, families and referral sources.
    • Updates job knowledge by participating in educational opportunities and completing 20 annual Continuing Education Units per FAC 65D-30.
    • Understands and follows all policies and procedures set forth by the Clinical Director, Executive Team, and President.
    • Adheres to all company policies and safety rules, guidelines & procedures.
    Necessary Abilities:
    • Understands the principles of psychiatric and addictions treatment
    • Ability to prioritize workload and work independently, and must be thorough and communicate effectively
    • Computer knowledge and use of computer systems a must
    • Ability to communicate effectively with staff, management, and clients
    • Knowledge of State & Federal Statutes Regarding Patient Confidentiality laws
    • Is knowledgeable of Clients Rights and treats all clients with dignity and respect in a consistent caring manner; adheres to federal confidentiality standards (HIPAA)
    • Must be reliable and consistent with attendance and punctuality to work
    Minimum Qualifications:
    • Bachelor's Degree, CAC, or CRSS Certification preferred
    • All employees may be required to work before, during and/or immediately after a natural or man-made disaster
    Salary Range:

    $43,000 / $50,000
