- Bachelor's degree required; or five years of relevant experience in the field of Long Term Services & Supports, which includes Developmental Disabilities.
- Driving is a requirement for this position.
- Prior case management and/or advocacy experience strongly preferred.
- Knowledge of forms and process for Long Term Medicaid services preferred.

Description
In order to be considered for any position at Foothills Gateway, Inc., we must have a completed Foothills Gateway application.
The Case Manager will be responsible for a variety of duties, which could include responding to contacts seeking information regarding services and supports for individuals with cognitive disabilities, assembling information about the individual for eligibility determination, providing Case Management services until the person can be admitted into services, being responsible for the planning, coordination, and monitoring of services and supports provided to individuals with cognitive disabilities and their families as appropriate.
Requirements:
$1700 hiring bonus after 90 days of employment. -1 full-time position at $ /hr. DOE/DOQ -Remote/Mobile position- must be able to attend trainings and meetings in Fort Collins, CO and throughout Larimer County.
Approximate accrual of 15 days per year paid time off to start, paid sick leave, and a full range of Medical, Dental, Vision, Life and Disability and other benefits.
