    We are currently looking for an experienced:

    HOME CARE REGISTERED NURSE (RN)

    Per Diem

    Job Description:

    • Actively engages in the interdisciplinary care planning process.
    • Delivers professional nursing services following the plan of care established by the Patient Care Coordinator.
    • Emphasizes positive outcomes through collaborative practice methods and the efficient use of resources.
    • Accurately documents to showcase proper delivery of home health services, aid in interdisciplinary coordination, support reimbursement, minimize legal risks, ensure compliance with regulations, and serve as a basis for quality improvement.
    • Displays effective interpersonal skills.
    • Demonstrates accountability for work assignments and professional development.

    Job Requirements:

    • Completion of nursing education from an accredited school of nursing.
    • Active RN license in the branch's state, with at least one year of clinical nursing experience in acute or long-term care; home health experience is desirable but not mandatory.
    • Current understanding of core medical-surgical nursing principles and practices.
    • Proficient clinical nursing abilities.
    • Familiarity with adult learning principles and medical record-keeping standards.
    • Effective verbal and written communication skills.
    • Proficiency in computer usage and familiarity with various applications.
    • Strong problem-solving, critical thinking, and reasoning skills for patient care planning and issue resolution.
    • CPR certification.
