    Summary



    This is a Direct Hire Authority (DHA) solicitation utilizing the DHA for Certain Personnel of the DOD Workforce to recruit and appoint qualified candidates to positions in the competitive service.

    About the Position: Employee will be working in the Department of Preventive Medicine, Traumatic Brain Injury Center at Madigan Army Medical Center.

    - A recruitment or relocation incentive may be authorized.

    -Salary negotiation may be available for those candidates who are new to Federal service



    Duties

    • Serve as a member of a multidisciplinary team to provide assessment, planning, implementation, coordination, evaluation, and monitoring of patients for health options and services.
    • Provide advanced practice clinical nursing, administrative and organizational skills in managing the continuity of care for the beneficiary populations.
    • Educate both the patient and members of the healthcare delivery team about case management, healthcare and treatment options, community resources, insurance benefits, psychosocial concerns, etc., so that informed decisions can be made.
    • Monitor exceptions, deviations, and differences from the established protocols to identify and report problems.
    • Conduct extensive evaluation of case management patients to establish in detail and specificity the nature of their continuity of care needs as well as causal and contributing conditions and circumstances.
    • Identify problems with health care access and utilization in both the military and civilian sectors and recommends alternatives to overcome these difficulties.
    • Serve as a supervisor and assign work to staff based on priorities, difficulty of assignments, and capabilities of employees. Provide oversight and rate employees.
