    Job Description

    A Nurse Practitioner or Physician Assistant is needed in Patchogue, New York. The primary focus of the provider is to deliver quality primary, preventative, and behavioral health services to all patients.

    • Valid New York NP or PA license


    • Board Certified as an NP/PA by a nationally recognized board


    • Spanish fluency is a plus
