Job Summary:
We are currently seeking a temporary Registered Nurse (RN) to join our Case Management team as a Day Shift Case Manager. In this role, you will be responsible for coordinating patient care, ensuring efficient transitions of care, and advocating for the needs of patients. The ideal candidate will have experience in case management and a strong commitment to patient-centered care.

Responsibilities:

• Conduct comprehensive assessments of patients' medical and psychosocial needs.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Coordinate patient care transitions, including discharge planning and referrals to post-acute care facilities.
• Communicate with patients, families, and healthcare providers to ensure continuity of care and smooth transitions.
• Advocate for patients' rights and preferences in healthcare decision-making.
• Monitor and evaluate patient progress and outcomes, adjusting care plans as needed.
• Collaborate with insurance companies and other payers to obtain authorizations for services and ensure reimbursement.
• Document all aspects of patient care accurately and in compliance with regulatory requirements.
• Participate in interdisciplinary team meetings and case conferences to review patient cases and develop care plans.

Qualifications:

• Valid Registered Nurse (RN) license in the state of practice.
• Minimum of one year of experience in case management or care coordination preferred.
• Certification in Case Management (CCM) or willingness to obtain certification within one year of hire.
• Strong assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to work effectively in a fast-paced environment and collaborate with a multidisciplinary team.
