Worldshine is a senior care management company with a passion for enhancing seniors' lifestyles through continuous innovations in care by providing high quality individualized services and specialty care for the elderly with disabilities and dementia. Worldshine is operating 3 adult medical day care centers located at Germantown, Beltsville, and Frederick serving about 500 participants. You will work alongside innovative leaders and you'll be a part of shaping our future in the years to come.

Job Description:

This position will work with clients, family members and other departments to identify necessary programs and services. The duties include comprehensive assessments, document notes in the participant records and meet the regulations and requirements.

Responsibilities for Social Worker

· Provide training for staffs

· Complete care plan, mental assessment and evaluation with score and signature simultaneously with 3-month schedule and 6-month schedule.

· Provide the psychosocial support as needed.

· Provide services to maintain mental health and well-being of clients

· Help with new participants admission

· Provide service skills and emotional support to staffs

· Conduct discharge plan, process, and referral options.

· Connect with county and state social services departments to update the social services policy

· Coordinate with the nurse, activity, or other department to reach a specific goal or mission.

· Coordinate and participate meetings as needed

· Participate in marketing activities as needed

· Other duties as assigned.

Qualifications for Social Worker

· Valid LBSW or LMSW required

· Strong critical and strategic thinking, analytical and planning skills

· Ability to juggle multiple tasks at once

· Excellent interpersonal skills

· Ability to create policies for employees to follow

· Strong English communication skills, bilingual preferred

· Have knowledge of state and county compliance regulations
