- Coordinate residential clinic operations and provide guidance on medications and health inquiries.
- Collaborate with residents and community resources to maximize wellness.
- Keep abreast of residents' functional status and provide updates to the Life Connect team.
- Maintain positive relationships with physicians and hospitals.
- Participate in orientation of new residents and coordinate educational programs for wellness promotion.
- Graduate of an approved nursing school with current RN license.
- Experience in assessing and intervening with seniors/geriatric clients preferred.
- Strong assessment and communication skills required.
- Current CPR certification and proficiency in relevant software programs.
