    Job Title: Licensed Practical Nurse LPN

    We are seeking a skilled and experienced Licensed Practical Nurse (LPN) to join our team in Skilled Nursing facilities. As an LPN with Health Advocates Network, Inc., you will have the opportunity to work with a dynamic team and make a meaningful difference in the lives of our patients.

    Job Summary:

    As an LPN, you will be responsible for providing high-quality patient care in a fast-paced Skilled Nursing facility setting. You will work closely with other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.

    Responsibilities:

    • Assess patients' physical and emotional needs and develop personalized care plans
    • Administer medications and treatments as prescribed by physicians
    • Monitor patients' progress and report any changes or concerns to supervisors
    • Collaborate with other healthcare professionals to ensure comprehensive patient care
    • Maintain accurate and detailed records of patient interactions and treatment

    Requirements:

    • A degree from an accredited school of nursing
    • An active, current LPN license in applicable state or reciprocity
    • At least 1 year of full-time LPN experience in a Skilled Nursing facility
    • BLS through American Heart Association or equivalent
