    We are actively looking for multiple continuous care positions available for full-time and per diem LPNs.

    Summary:
    In this role, you will be responsible for assisting in the nursing care needs of our patients as directed by the RN and outlined in the care plan.

    Responsibilities include, but are not limited to:

    • Assessing patient/family response to care and changes in condition.
    • Communicating changes to the RN case manager and implementing necessary adjustments in the treatment plan.
    • Administering appropriate nursing procedures and techniques.
    • Providing support for end-of-life care activities under the guidance of an RN or physician.
    • Documenting clinical observations, interventions, and outcomes accurately and in a timely manner.

    Qualifications:

    • Graduate of an accredited practical nursing school.
    • Current LPN licensure from the state board of FL.
    • At least 6 months of general nursing experience preferred, including exposure to hospice, home care, med/surgery, or critical care.
    • Strong nursing judgment and documentation skills.
    • Effective oral and written communication abilities.
    • Preferred current CPR certification.
    • Demonstrated empathy towards patients and caregivers.

    Ability to collaborate effectively within a healthcare team.
