- Perform continuous assessments of patients to identify their psychosocial, financial, environmental, and community resource requirements.
- Collaborate with the care team, physicians, payors, patients, and families to address care needs and fulfill the patient's care plan.
- Provide top-notch services as prescribed by the physician, inclusive of family counseling and caregiver education.
- Thoroughly record daily care delivery in our EMR system.
- Demonstrate compassion in delivering care, with a focus on achieving tangible results.
- Access to comprehensive health, dental, and disability benefits.
- Masters or Doctoral Degree from an accredited school of Social Work.
- Hold a valid Social Worker License, state driver's license, and auto liability insurance.
- Minimum of one year of experience as a Masters Social Worker in acute care, rehabilitation, or home health settings.
