    Agency Intake Specialist

    $14.25-$19.76-$25.27

    Monday-Friday 7:30 am-4:30 pm, Flexible

    Job Summary: The Intake Specialist helps connect individuals and families to desired services. Communicates frequently with stakeholders, parent/guardians and other entities regarding services. Coordinates client transitions, meetings, visits, and service requests with the appropriate department head. Works with the Business Development Director to identify new program opportunities based on referral feedback. Connect new and existing referrals to program opportunities within the agency. Identify new program opportunities based on client and family requests.

    Job Requirement: The employee will be expected to represent the corporation with a professional carriage and demeanor. Adhere to agency policies and procedures regarding confidentiality, ethics, rights and dignity of individuals with disabilities at all times. Knowledge of the structure and content of the English language, including proper spelling, grammar and composition, for purposes of speaking and writing in order to: follow and/or convey oral written instructions or information, complete required forms and documentation and knowledge of basic math skills. Ability to perform the following: decision making, analyzing, reasoning, reading, talking, hearing, seeing, writing, driving, reaching, pulling, pushing, toileting, bathing, lifting, seeping, bending, stooping, twisting, and climbing stairs. Basic knowledge of local community organizations, activities and events. Maintain an even temperament during times when a person served is upset or aggressive. Must have dependable transportation, a valid driver's license, proof of auto insurance in the employee's name and willingness to provide transportation in personal automobile unless a waiver is approved by department head and HR. Willingness to work non-standard/flexible hours, including other locations, evenings, weekends and holidays. Accessible to management during non-working hours via a telephone, email or some other timely electronic method. Attend required meetings and complete required training timely, which may occur outside regularly scheduled work hours. Ability to use basic computer functions, such as work processing and email, and a calculator depending on the job setting. Ability to physically redirect and assist a person served as prescribed in a person's behavior management plan or during an emergency situation. May be required to use a Hoyer lift or other assistive devices. May be required to drive a company vehicle, such a as a modified van with a lift.

    Job qualifications: Must be 18 years of age, high school diploma or general equivalency required. Experience in the field and knowledge of the community are preferred.
