- Full-Time: Mon-Fri 8:30am-5:00pm (Flexibility is key!)
- Deliver compassionate care to residents
- Perform resident assessments and create personalized care plans
- Collaborate with the healthcare team for effective care coordination
- Ensure accurate submission of Medicare Program
- Provide ongoing education to the Interdisciplinary Team
- Active RN licensure
- Minimum of two years' experience in Medicare/PPS and OBRA MDS
- Strong attention to detail and organizational abilities
