JOB SUMMARY:

Administers professional nursing care for patients in a primary care setting (home or alternate-site) following nursing practices, agency policies, and regulations. Delivers nursing services to patients and families in line with the RN scope outlined by the North Carolina Board of Nursing.

MAJOR JOB RESPONSIBILITIES:

1. Plan, provide, oversee, and document professional nursing care utilizing the nursing process for patients as per the NC nurse practice act, physician orders, and established policies and procedures. Utilizes professional nursing judgment to customize the care plan based on the patient's baseline needs and response to care.
2. Employ comprehensive assessment skills to prioritize and establish realistic outcomes; collaborate with the healthcare team to assess the effectiveness and cost efficiency of patient care.
3. Evaluate educational requirements of patients/families regarding health changes, disease progression, and care plan.
4. Devise and execute an efficient discharge plan for patients right from the onset of services.
5. Source or create suitable educational materials for patients/families; devise and implement appropriate educational interventions; evaluate the effectiveness and outcomes of patient/family education.
6. Exhibit professional nursing behavior through demeanor, appearance, communication, respect, ethical decision-making, critical thinking, and problemsolving skills.
7. Engage in weekly interdisciplinary team meetings and case conferences, as needed, to ensure adequate care and service delivery to the patient.
8. Stay updated on Medicare/Medicaid/JCAHO standards through attending inservices and workshops.
9. Assess the severity of patient symptoms, notify the physician, record significant findings, and intervene as necessary.
10. Record care and services provided in compliance with DHCH Policy and Procedures and regulatory standards.
11. Accurately, promptly, and concisely complete documentation.
12. Conduct medication reconciliation and education to prevent adverse events and hospital readmissions.
13. Observe patient responses to medications, report noteworthy incidents to physicians.
14. Regularly communicate with team members, caregivers, and physicians.
15. Adhere to infection control precautions and use necessary personal protective equipment.
16. Delegate and oversee care provided by NCA's/LPN's.
17. Create care plans for aides, assign care based on aide skills, availability for consistent patient care, patient preference (when feasible), and other factors dictated by patient care requirements.
18. Identify personal learning needs and take corrective action; assist in planning and conducting staff development programs to enhance DHCC efficiency.
19. Proactively engage in professional development by upholding necessary competencies, identifying learning needs, and seeking appropriate training or education opportunities.
20. Implement agency policies, procedures, and goals.
21. Collaborate closely with agency billing staff to ensure billing accuracy.
22. Maintain confidentiality of employee and patient data.
23. Promote a harmonious, positive workplace atmosphere.
24. Maintain a polished, professional appearance.
25. Participate in Performance Improvement Program.
26. Foster quality, comprehensive services through teamwork.
27. Perform other assigned duties.

WORK ENVIRONMENT AND REQUIREMENTS:

Deliver patient care primarily in the patient's home or skilled nursing facility (>50%)

Travel to and from patients' residences

PREFERRED QUALIFICATIONS

Education and Formal Training

• Associate Degree or Diploma in Nursing compulsory
• Registered Nurses are encouraged to enroll in a DUHS approved BSN program after completing two years of service from their start date.
• ASN, Diploma, and BSN prepared nurses can join the DUHS RN Clinical Ladder program

Licenses, Certifications

• Must possess an active RN license in North Carolina or Compact RN license in eligible states
• Current State of North Carolina Cardio-Pulmonary Resuscitation Certificate (CPR) (American Heart Association only) (may be renewed during orientation as necessary)

Professional Work Experience & Requirements

• 1-2 years of med-surgical, critical care, or home health/hospice/infusion experience preferred
• Valid driver's license and current auto insurance coverage
• Prior experience in Home Health, Hospice, or Infusion desirable

Knowledge, Skills & Abilities

• Familiarity with Medicare and Medicaid regulations, JCAHO and DFS standards, ICD-9 Coding.
• Understanding of the roles of registered nurses, licensed practical nurses, and NCA
• Familiarity with the nursing process and its appropriate application
• Capability to assess nursing needs of acute and chronic patients and their families independently
• Ability to proactively seek resources and collaborate with other professionals
• Establish and maintain effective work relationships
• Clear communication skills with patients, families, medical staff, administrators, leaders, and others
• Capability to educate patients and families according to the care plan
• Utilize sensory and cognitive functions to process, prioritize information, treatment, and follow-up
• Fine motor skills and accurate documentation abilities
• Competence in using computers and learning new software
• Ability to document relevant information using computer or paper tools
• Proficiency in wound identification and treatment, infusion, and other advanced nursing skills
• Critical thinking and organizational skills
• Understanding of infection control protocols and safety measures
• Strong computer skills including MS Outlook, Word, and Excel
• Excellent rapport and communication with patients and their families
• Capability to work independently in diverse environments
• Able to endure long drives, stooping, bending, lifting, and climbing stairs
