Home Health Registered Nurse (RN) responsibilities include:
- providing services in accordance with the plan of care
- making the initial evaluation visit and regularly reevaluating the patient's nursing needs, as well as ensuring the plan of care needs are met and making any necessary revisions
- monitoring and evaluating patients' progress and updating care plans accordingly
- educating patients and their families on managing the conditions and promoting wellness
- informing physician and other personnel of changes in the patient's condition and needs, as well as collaborating with interdisciplinary healthcare team members to coordinate the best possible care
- taking on-call duty nights, weekends, and holidays, as assigned
- participating in in-service programs, supervising and teaching other nursing personnel as necessary
Requirements for the Home Healthcare Registered Nurse position:
- valid Registered Nurse (RN) license in the state of Illinois
- minimum of one year of nursing experience, preferably in home health or a related setting
- proficient in using electronic medical record systems, such as Axxess
- previous experience in geriatrics and home health
- ability to work independently and prioritize tasks effectively
- ability to provide care with empathy, respect, and understanding
- strong communication and interpersonal skills
Job Type: Per Diem
Pay: Varies. Pay per visit depending on Home Health experience
