The Remote Night Float Advanced Practice Practitioner will provide after-hours telehealth care and access to comprehensive health care for the individual and family across all ages and genders within the context of Patient Centered Medical Home (PCMH) policies and procedures.
Responsibilities
- Works as an integral member of the care team which includes careful attention to customer service population management appropriate delegation participation in health centers quality initiatives
- Conducts a program of care within their discipline's scope of practice in collaboration with primary care teams supervising physician provider when warranted with responsibility for examination diagnosis treatment patients Obtains history physical examinations patients ambulatory setting Utilizes laboratory diagnostic services appropriate At direction Chief Medical Officer Senior Medical Director works patient access normal business hours coverage after-hours OCHS patients This done rotating basis NB After ours provides access clinical triage emergency advice referral urgent emergency department outside regular operating hours When night float team fully staffed night float advanced practitioner provide scheduled Telehealth alternating week night call weekend call Be assigned other clinical support work primary teams Epic desktop management PCPs especially those ETO direction Chief Medical Officer Senior Medical Director Renders service OCHS competent professional ethical manner make diagnosis administer treatments evidence-based medicine whenever possible always accepted standards liPrescribes buprenorphine medications needed safe effective patient living opioid use disorder Seeks outside advice consultation necessary Refers specialty confirmation diagnosis treatment appropriate Experience willingness practice team-based are Participates health promotion disease prevention activities including screening immunization events sponsored participates quality assurance assessment activities relies on each measure continuously improve quality healthcare Adheres documentation set for staff Performs emergency consistent established policies procedure standards accepted Care Participates complies all policies related MassHealth state federal reimbursement programs commercial payors contract affiliate In accordance Massachusetts law committed wage transparency salary range position $100000 year Actual compensation vary factors such experience qualifications internal equity jobs criteria
