Job summary
Mountain View Health and Rehabilitation Center is currently seeking a Social Services Director to assist with maintaining the social service program designed to meet the individual physical, mental, and psychosocial needs of the residents.
Essential Functions
- Works with residents, families, and community support to complete social service history, discharge plan, and development of interdisciplinary care plans designed to meet the bio-psychosocial needs of resident.
- Assisting residents and responsible parties in location and using financial legal mental health other community resources.
