    Responsibilities

    The Care Management Associate (CMA) functions as a patient advocate by collaborating with the multidisciplinary care team to facilitate timely and safe transitions of care.

    The CMA works closely with the care management team as well as the medical and nursing staff to facilitate the coordination of post-acute services.

    Norton Healthcare is committed to establishing health equity in every neighborhood, and Norton West Louisville Hospital is an example of that commitment put into action.

    Opening November of 2024, this state-of-the-art facility is being built at 850 South 28th Street and will offer comprehensive services, including a 24-hour emergency department, inpatient care, operating rooms and imaging technology.

    It will serve both adults and children. It's designed with the voice of the community in mind, and will change health care for generations to come.
