Job Overview:
Evaluates patients, plans for and provides therapeutic interventions in a transdisciplinary/team approach. Responsible for the communication and coordination of treatment programs with other team members including the patient/family.
Provides work direction to assistants, interns and other support personnel, participates in program developments, continuous improvement and clinical education; and provides in-services to staff, hospital personnel and the community.
Will also be expected to be a member of a collaborative team which is patient focused, functional outcome based and patient/family centered.
Job Requirements:
Bachelor's Degree
Basic Life Support for Healthcare Providers (BLS), State Licensure
experience in a related field
Job Responsibilities:
Actively participates as a member of the Multidisciplinary Team as evidence by: identifying participant/caregiver needs; recommending interventions, and collaborating with team members for mutually determined solutions
Conducts and documents comprehensive geriatric assessment of physical condition and functional status.
Demonstrates knowledge and application of the principles of customer service and PACE regulations
Facilitates the development of plans of care that addresses ambulation and functional needs, provides physical, establishes goals and instructs the participant and his/her family in relevant methods of care.
Manages caseload efficiently and is able to prioritize competing requests for therapy. Able to collaborate with other therapists in order to satisfy service requests in a timely manner.
