    Description

    Job Title: Registered Nurse

    The Registered Nurse provides quality nursing care, education and patient advocacy and clinical case management at UpHams Corner Health Center.

    About the Role

    • Delivers patient care and patient education in a manner that is appropriate to patient's age, physical ability, educational level and culture.
    • Participates in regularly scheduled team and department meetings.
    • Provides case management for the addiction medication program and for the patients in recovery.
    • Coordinates follow-up of abnormal lab results and communicates with laboratory, radiology, urgent care and other departments.

    Responsibilities:

    • Maintains adequate supply of vaccines, immunization information sheets and medications.
    • Documents all nursing assessments and interventions using SOAP note format.
    • Ensures that all diseases cited in the Massachusetts Department of Public Health list of Reportable Diseases are reported to the Local Board of Health.
    • Communicates clearly with patients and with staff.

    Qualifications:

    • Licensed Registered Nurse (B.S.N. preferred) in Massachusetts.
    • Two years of nursing experience, preferably in an ambulatory care or community health setting.
    • Experience with addiction services and bilingual language skills in Spanish or Portuguese Creole desirable.
