    POSITION SUMMARY

    In response to COVID-19 Pandemic, Oregon's Department of Human Services temporarily allows PersonalCare Assistant to provide nursing care according the emergency rule NF Provider Alert- Revised(October 12, The Personal Care Assistant provides quality nursing care to residents; implementsspecific procedures and programs; coordinates work within the department, as well as with otherdepartments; reports pertinent information to the immediate supervisor; responds to inquiries orrequests for information; assists the immediate supervisor with tasks to support department operations.

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    1. Provides quality nursing care to residents in an environment which promotes resident rights,dignity, freedom of choice, and their individuality while providing individualized attention andencouraging residents to maintain or attain the highest practical physical, mental, andpsychosocial well-being. Answers residents' call lights promptly and courteously.

    2. Ensures that individualized care plans are followed by providing the charge nurse or other careplanning staff with specific information, changes and observations of the residents' needs /preferences.

    3. Attends to the individual needs of residents including assistance with grooming, bathing, oralhygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring,ambulation, range of motion, communicating, or other needs in keeping with the individuals' careplan requirements.

    4. Provides care that maintains each resident's skin integrity to prevent pressure ulcers, skin tearsplus other damage by changing incontinent residents, turning, repositioning immobile residents,and applying moisturizers to fragile skin.

    5. Protects the personal belongings of each resident including eyeglasses, dentures, hearingaids, furnishings, jewelry, clothing, memorabilia, etc. Promptly reports missing itemsaccording to established policy.

    6. Performs various tasks assigned by the charge nurse including vital signs, residents' weights,applying creams/ointments, collecting specimens, as well as lifting, moving, and transportingresidents, using proper body mechanics or lifting devices for accident prevention.

    7. Responsible for keeping records documenting care provided or other information in keepingwith department policies while performing all job responsibilities in accordance withprescribed safety and infection control procedures including thorough hand washing, use ofdisposable gloves where indicated, and proper disposal of soiled materials.

    8. Communicates and interacts effectively and tactfully with the resident, visitors, families, peers, andsupervisors.

    9. Attends and participates in resident activities and the execution of the Expressions of My Lifeprogramming for Expressions memory care residents. Incorporates the Life Story and 100 ThingsAbout Me information into routine tasks and ADL's for Expression memory care residents

    10. Responsible for following universal precautions and safe work practices, establishedfire/safety/disaster plans, risk management, and security, report and/or correct unsafe workingconditions, equipment repair and maintenance needs.

    11. Completes requirements for in-service training, acceptable attendance, dress codes includingpersonal hygiene, and other work duties as assigned.12. Cooperates and works together with all co-workers, planning and completing job duties withminimal supervisory direction, utilizing appropriate judgment.

    13. Performs other duties as assigned.

    QUALIFICATIONS AND EXPERIENCE

    Upon hire, completion of required online training course and associated skills competency checklistdeveloped by the American Health Care Association in association with Academic Platforms, LLC(branded as ). Completion of required supplemental training regarding infectionprevention and feeding assistant. Previous experience with caregiving or customer service preferred.

    EDUCATIONHigh school diploma or equivalent.

    SUPERVISORY RESPONSIBILITIESThis position does not have supervisory responsibilities.

    LANGUAGE SKILLSAbility to communicate effectively, in English, both verbally and in writing, with personnel at all levels ofthe organization, residents and their families, and members of the community.
