- Perform a nursing assessment- to include demographics, medical/physical status, and mental/emotional/behavioral status
- Triage all medically related telephone or telehealth encounters
- Recognize and interpret symptoms of the patient's condition
- Perform triage and examination
- Observe and correct, if possible, environmental factors dealing with comfort and safety of the patients
- Prepare patients for physicians' examination, draw routine laboratory studies as required, administer injections, assess vital signs, measure height and weight, perform skin testing and provide immunizations as ordered
- Conform to nursing techniques and procedures and established standards and policies
- Administration of treatment as directed by PA Thrive Partnership HIV medical specialists
- Provide new patient education to patients and family
- Conduct patient and/or family education in an ongoing fashion following the educational protocol
- Assist with the coordination of transportation efforts with the appropriate case manager to ensure the patients have access to care
- Develop a plan of care with the patient based on the patient's individual needs, available community resources, and financial resources
- Provide nursing services to individuals who have been diagnosed with an STI to include access to treatment, and treatment and prevention education.
- Assure patient's care and condition is monitored by frequent phone calls and office visits
- Evaluate the effectiveness of the current medical treatment, and notify the medical specialists if there are concerns
- Reassess the patient at established intervals to:
- Expanded work hours
- Travel to remote clinical sites on clinical days, leaving before scheduled work hours and returning after scheduled work hours, earning compensation time
- Attend educational sessions requiring travel to begin and end outside the usual scheduled workday
- Lifting/carrying/transporting heavy clinical equipment and charts
- The ability to lift/carry and or transport chart bags and clinical equipment weighing up to 50 pounds. This includes up and down no less that one flight of steps

Job Summary / Basic Function
The HIV/HCV and Sexually Transmitted Infection Nurse specialist will work under the direction of the Supervisor of Clinical Operations and the HIV medical specialists by coordinating the patient's care following the established protocol at PA Thrive Partnership, a Ryan White Program housed in the northwest region of Pennsylvania. The duties include performing direct patient care, phlebotomy services, administering treatment as prescribed, adherence counseling, HIV counseling and testing services, and offering guidance and direction to patients and family members.
Typical Work activities:
1.) Assessment
- Evaluate the effectiveness of the care plan in meeting predetermined goals
- Determine if services continue to be cost effective for the patient
- Revise goals and/or plan of services accordingly.
- Establish and maintain up-to-date patient records including assessment, service plan implementation and evaluation of care
- Submit data reports, at designated intervals, using a standard format for program monitoring and evaluation
- Follow Up
- Follow up with the patient on a monthly basis and or prn either by phone, telehealth or in person
- Discharge Planning
- Prepare patient for discharge from the clinical program when services are no longer required or requested. Make arrangements for any post-discharge ongoing support/direct care services for the patient
4.) Travel to Rural Health Care Clinics
- Coordinate the recommendations of the HIV medical specialist with other members of the health care team, and report changes in the patient's condition as indicated
- Coordinating lab work, as per the medical specialist's directive. Laboratory referrals will be made to local hospital and/or reference labs. Lab work will be obtained prior to clinic visits for the physician medical specialist's review
- Abide by the policies surrounding Health Insurance Portability Accountability Act (HIPAA) of 1996, and Act 148 /Act 59 of 2011, an amendment to Pennsylvania's Confidentiality of HIV-Related Information Act (commonly known as Act 148).
- Supply de-identified data for use in clinical quality initiatives (CQI), benchmarking, as well as measurements for quality of care markers
- Participate in the agency quality management team and projects
- Facilitate improved health outcomes as a result of quality indicators
- Check equipment in the HIV clinic to assure it is in proper working order
- Participate in emergency drills and maintain current Nursing License in Pennsylvania
- Dispose of biohazardous waste in appropriate waste containers
- Perform basic office management duties, such as: answering phones and directing calls, scanning and faxing documents, checking inventory, assisting in planning efforts, developing policy and procedures etc.
- Complete other tasks as assigned by the Executive Directive, HIV Medical Director, HIV medical specialists, or the Supervisor of Clinical Operations
7.) Counseling and Testing
- Educate patients living with HIV/AIDS and Hepatitis C (HCV) about the disease process, treatments, and primarily medications adherence
- Provide written, verbal and audio media to assist learning
- Individualize the teaching plan per the patient's needs
- Provide the co-infected patient support during HCV treatment
- Facilitate treatment under the direction of the medical specialists
- Provide Education to the patient and family surrounding the disease process and prevention techniques
- Initiate required referrals, and facilitate referrals made by the clinics sub-contracted by PA Thrive Partnership
- Track referrals from initiation through completion and report those activities to the Supervisor of Clinical Operations
- Monitor and re-assess patient follow through with appointments/services retention in HIV care
