    Description
    SPECIFIC RESPONSIBILITIES
    • Act as liaison to medical providers and pharmacies.
    • Coordinate medical appointments.
    • Make appointments for outside services as indicated, check progress notes.
    • Dispense medication, pick up and implement orders from Psychiatrist& MedicalDoctors.
    • Ensure medication compliance, review MAR,and ensure clients have medication refills available.
    • Medical chart maintenance as required by regulatory and oversight bodies.
    • Conduct physical screenings of children.
    • Prepare list for infection control, order medical supplies.
    • Provide in-service training to residential counselors.
    • Provide family planning and health information to clients in individual and group settings.
    • Escort residents to appointments as needed.
    Perform all other duties as assigned.

    QUALIFICATIONS

    Experience required:
    • Experience working with or knowledge of the unique health care needs of Lesbian, gay and transgenderyouth.
    • Knowledge of and /or experience working with LGBT youth-oriented health care providers in the community
    • Acceptable background clearance, including fingerprinting.
    • Must be able to commute between Brooklyn and Queens.
    • NY/NJ/CT driver's license with a clean driving record.
    Education:
    • Minimum of a High School Diploma, associate degree or above preferred.
    • Current NY State LPN Certification required.
    RELATIONSHIP WITH OTHERS

    Externally: interacts with NYC Department of Correction, NYC Department of Education, and other community-based organizations

    Internally: interfaces with other service programs as well as business support services

    • Possess a strong belief in people's ability to grow and change; forge a mutually respectful partnership with persons served and their families.

    • Ability to set limits and maintain helping role of practitioner and to intervene appropriately.
