- Associate or bachelor s degree in Nursing Minimum of two years experience working as a psychiatric nursing in a community based behavioral health setting Valid and current Rhode Island Registered Nurse License
- Valid driver's license and reliable transportation.
- Familiarity with screening/ assessment tools, local care coordination resources and evidence-based practices
- Oral and written communication skills at a proficient level ensuring effective and professional communication
- Ability to provide crisis intervention brief counseling or case management when determined necessary. Ability to provide psychiatric/medical consultation to HT staff and/or community providers as appropriate to position.
- Bilingual skills are compensated by an additional 6%, above base pay.
- Travel to and from clients' residence, community locations and office site, which could include using walkways, stairs and/or elevators.
- Ability to lift up to 20lbs.
- Must be able to work remotely and in person adhering to PHI requirements.

Position Summary: The Registered Psychiatric Nurse (RPN) provides screening assessment and psychiatric nursing services on the Healthy Transitions Team (HT). This multi-disciplinary team is designed to provide outreach comprehensive evidence-based treatment referral and support services for a population of youth and young adults and their family members struggling with severe mental illness and co-occurring substance use. The goals of the Healthy Transitions Team Nurse are:1) To assist clients with meeting personal objectives for improving and maintaining mental health especially regarding medication management and compliance; 2) Identify and provide intervention for youth and their families who are experiencing or at risk of developing substance use disorder; and 3) Promote improve and sustain the overall wellness and physical health of the target population.
