    Position:
    Registered Nurse (RN) / Licensed Practical Nurse (LPN) (Home Health)

    Status:
    Full-Time or Part-Time

    Location:
    Jefferson Hills PA, Pleasant Hills PA

    Role Summary:
    • The RN will oversee the home care of chronically ill patients through a disease management process, OASIS assessment, plan of care, and established protocols.
    • Responsibilities include educating patients/caregivers on illness management and promoting home health support systems to reduce re-hospitalization and emergency service utilization.

    Requirements:

    EDUCATION:
    Licensed RN, BSN, or LPN with current PA state license.

    EXPERIENCE:
    Previous experience in Home Health preferred. Ability to work independently and collaboratively within a team. Must have a valid driver's license.


    Company Fleet Vehicles Available: Equipped with a gas card, maintenance, repairs, inspections, and insurance coverage for your convenience and efficiency.

    Job Types: Full-time, Part-time

    Pay Rate: $120.00 per hour
