    Description

    The Licensed Practical Nurse will:

    • provide direct support and nursing care to people with disabilities for routine health care, conduct various assessments, address minor illnesses/injuries and ensure proper follow-up and documentation.
    • participate in the ISP planning and monitoring as needed and supplement the services of the RN as needed.
    • provide requested training for DSP as required and may provide newly hired LPNs with shadowing training.
    • make referrals to other medical professionals and facilities as appropriate.
    • oversee services to people supported as well as the activities, meetings, and specific healthcare needs.
    • attend staff meetings and trainings.
    • provide program support.

    Requirements

    • Be a graduate of an accredited school of nursing with registration and license (LPN) to practice nursing in the District of Columbia and one year nursing experience.
    • Be able to read, write, and speak the English language and have the ability to communicate effectively with the people to whom services are provided.
    • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
    • Ability to write routine reports and correspondence.
    • Ability to perform basic computer and office equipment skills.
    • Ability to effectively present information in one-to-one and small group situations to people supported, families, guardians, and other medical professionals.
    • Ability to coordinate and/or prioritize needs of different program sites.
    • Ability to document all healthcare and medical needs as required by all licensing and monitoring entities.

    • All Volunteers of America Chesapeake orientation components
    • All requirements as defined by the DC DDA and other monitoring entities
    Note: All employees must maintain all on-going training, certifications, and be otherwise compliant and in good standing with all program requirements as a condition of continued employment. Additionally, all LPN Coordinators will keep their registrations and license(s) in good standing at all times. OTHER:

  • Annual Negative TB Screening and Health Certificate clearance
  • CPR/First Aid Certification must be acquired and maintained in good standing once employed
  • Driving: Employees must be privileged to drive according to the criteria set forth by VOAC MVR guidelines. Employees must be approved to drive by the VOAC Baseline Criteria and Driver designation review criteria listed below.
  • Possess a valid driver's license in jurisdiction of residence and have been licensed for at least 3 years
  • The Recommended Driver age is between the ages of 25 and 70 to transport people receiving services in company vehicles. (Drivers between the ages of 21 and 25, in order to drive a company vehicle, must have a clean MVR. These staff members are NOT permitted to drive 15 passenger vans.)
  • Company vehicles may include cars, mini-vans, trucks, wheelchair accessible vans and 12 & 15-passenger vans.
  • RNs are not permitted to drive their own car to transport people receiving services.
  • PRINCIPLE ACTIVITIES:
    • Be available, on-call, as scheduled, for nursing consultation and/or treatment of minor illnesses or injuries and render nursing assistance in medical emergencies.
    • Develop rapport with each person receiving nursing care. Demonstrate respectful interactions at all times.
    • Participate in the pre-admission evaluation, program design and placement of each person admitted to the program.
    • Prepare, as required, Medication History at admission and perform periodic re-evaluation, monthly medication administration review and quarterly systems review.
    • May participate as a member of the IDT providing general health counseling, family planning, general sex education and personal hygiene at the appropriate. developmental level of each individual person.
    • Provide clarification of presented information in a method best understood by the person and his/her family.
    • Provide appropriate in-service training for program staff in basic health care and hygiene skills.
    • Coordinate medical appoints, transportation and follow-up.
    • Provide instruction to staff as related to medical care - operate side-by-side with direct support professionals.
    • Assess, plan and follow-up on medical issues noted with people to include location of needed medical resources in the community, as appropriate.
    • Complete the following specific on-going tasks:
    • Review and document in the staff log book for health concerns and complaints.
    • Make appropriate entries in nursing notes relating to individual care.
    • Review medication administration instructions on Medication Administration/Observation. Record for correct transcription.
    • Review Medication Administration/Observation Record routinely and file in the appropriate files.
    • Audit medical books as required.
    • Review weight gain/loss monthly, including arrangements with dieticians/nutritionists and record monitoring compliance with recommended diet.
    • Prepare medical summaries and attend ISP meetings as needed.
    • Complete annual assessments as needed.
  • Communicate with RN regarding each person's medical needs including routine medical needs and emergency treatments.
  • Reviews Health Passport to make sure information is correct.
  • Composes monthly nursing report.
  • Provide appropriate in-service training to staff in basic health care and hygiene skills.
  • Supervise and support DSPs in the provision of medical support for the person.
  • Maintain up-to-date knowledge of Volunteers of America Chesapeake's policies and procedures.
  • Be knowledgeable of the rules and regulations of the relevant service environments including ICFs, Waiver, and Res Hab and adhere to the program guiding principles.
  • Contribute meaningfully during internal audits and external monitoring visits including: Evans Class Monitors, Quality Trust, HRLA, DOH, PCR, DDS, DDA in the District of Columbia.
  • Complete all necessary requirements related to MCIS-generated issues within the designated timeframes.

    • The ability to safely operate a motor vehicle to transport oneself, people and program supplies as necessary.

    • The physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear and perform basic and light home maintenance activities, and operate office equipment.
    • The physical ability to move and lift light objects up to 30 pounds such as mail, supplies, files, and equipment.
    • The physical ability to lift, transfer, carry, push, pull a person served when required through routine assistance or during an emergency.
