- Assess the overall status of the patient?s health condition and evaluates medication profile for optimal patient outcomes. Monitors therapy and goals of care. Documents all activities and communication as appropriate.

Are you looking for an opportunity to directly impact patient care? As a Clinical Pharmacist Specialist, you will provide pharmaceutical services to patients through daily rounds with a multidisciplinary medical team.
PGY1 residency required. (PGY2 in oncology/ambulatory experience preferred)
