Copy of Transcript (Showing all classes and date degree was granted (Associate's Degree or higher with a minimum of 12 Child Development units, including core classes, with grade of C or better))
Credential Copy (Copy of Child Development Permit or higher showing current status.)
Letter(s) of Recommendation (One personal letter and one professional letter required)
Resume
