    Description

    A currently competent, bachelor level professional who delivers behavioral health care services to residing in the community and, in the homes of trained providers. The counselor provides training and support in skills development , medication management, work readiness activities, case management and service coordination to clients experiencing significant disturbances in daily living due to severe and persistent mental illness. The counselor is involved in the recruitment, training and the provision of ongoing monitoring of care provided by therapeutic foster care providers. The counselor interacts regularly with providers regarding client issues and the provision of care; and counselor will monitor the completion of provider paperwork documentation. The counselor will assess needs, complete required documentation, case management, coordination with collateral care providers, advocate for client's needs, provide individual therapeutic rehabilitation skill development, illness management and recovery, (and family intervention as indicated), provide crisis assessment and intervention. The counselor will provide termination planning, and provide medication and psychosocial education to clients. The counselor will carry and respond to a pager providing on-call services. The counselor will be involved in the planning and carrying out of quarterly therapeutic provider meetings. The counselor will facilitate client transportation and recreational needs. The counselor functions as a member of the clinical treatment team. Performs other duties as from time-to-time, be deemed appropriate by the supervisor.

    QUALIFICATIONS

  • A bachelor's degree from an accredited college or university in the field of social work, human services, counseling, psychiatric rehabilitation, psychology or criminal justice is preferred.
  • In lieu of a bachelor's degree, the applicant should possess a license as a RN.
  • A minimum of 2 years working in a community based behavioral health setting.
  • Valid driver's license in the state of residence plus three-year driving history and a driving record in good standing.
