- Must have knowledge and must comply with state and federal statutes, rules and policies that affect the target population.
- File Short-term disability and FMLA
- Complete assessments and notes for each session with patients
- Complete weekly individual case management sessions
- Schedule outside appointments and setup transportation
- Ensure all medical follow up is completed and any necessary documentation is uploaded into the patient chart
- Complete Prior Authorizations for services or medications as soon as initially indicated, and subsequently prior to expiration, in order to ensure continuity of care.
- Must have excellent organizational, time management, and writing skills

Job DescriptionBenefits:We are a private substance abuse treatment center looking for a Case Manager to eliminate any barriers to patients remaining in treatment. We are small, intimate, and a family type treatment center looking for a permanent addition to our wonderful team. The ideal candidate is organized, humble, tactful, punctual, and coachable. The ideal candidate also has strong documentation skills and is a self-starter.
The Responsibilities include:
ORGANIZATIONAL SKILLS: Must be organized and able to schedule workload appropriately without hindrance to client appointments etc.
DECISION MAKING: Must be able to work independently with competent decision making using established criteria and guidelines.
Job Type: Full-time This is a Monday-Friday position 9:00am-5:00pm.
