- Coordinate and facilitate care for patients within the community care setting, adhering to Mission Act standards.
- Collaborate with interdisciplinary teams to develop and implement comprehensive care consult management for patients.
- Conduct thorough assessments of patient needs within the H&P, including physical, emotional, and social factors.
- Utilize multiple electronic systems simultaneously to document patient information, track care consults, and communicate with team members.
- Provide education and support to patients and their families regarding healthcare resources in the community, treatment options within the VA, and self-care techniques.
- Act as a liaison between patients, healthcare providers, and community organizations to ensure continuity of care and returning care back to the VA..
- Monitor patient progress and outcomes in the community and coordinating care back to the VA.
- Demonstrate strong verbal and written communication skills when interacting with team members, patients, and external stakeholders.
- Utilize critical thinking skills to assess complex situations, prioritize tasks, and make informed decisions regarding patient care.
- Adhere to ethical and professional standards of practice, maintaining confidentiality and respect for patient autonomy at all times.

The LTC Charles S. Kettles VA Medical Center is seeking a Full-Time Registered Nurse to serve as the Community Care Coordinator.
Duties
The Community Care Nurse is a registered professional nurse, qualified by education and experience.
Job Duties for Community Care Registered Nurse Care Coordinator:
Pay: Competitive salary, regular salary increases, potential for performance awards
Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA
