    Summary



    The LTC Charles S. Kettles VA Medical Center is seeking a Full-Time Registered Nurse to serve as the Community Care Coordinator.



    Duties


    The Community Care Nurse is a registered professional nurse, qualified by education and experience.

    Job Duties for Community Care Registered Nurse Care Coordinator:

    1. Coordinate and facilitate care for patients within the community care setting, adhering to Mission Act standards.
    2. Collaborate with interdisciplinary teams to develop and implement comprehensive care consult management for patients.
    3. Conduct thorough assessments of patient needs within the H&P, including physical, emotional, and social factors.
    4. Utilize multiple electronic systems simultaneously to document patient information, track care consults, and communicate with team members.
    5. Provide education and support to patients and their families regarding healthcare resources in the community, treatment options within the VA, and self-care techniques.
    6. Act as a liaison between patients, healthcare providers, and community organizations to ensure continuity of care and returning care back to the VA..
    7. Monitor patient progress and outcomes in the community and coordinating care back to the VA.
    8. Demonstrate strong verbal and written communication skills when interacting with team members, patients, and external stakeholders.
    9. Utilize critical thinking skills to assess complex situations, prioritize tasks, and make informed decisions regarding patient care.
    10. Adhere to ethical and professional standards of practice, maintaining confidentiality and respect for patient autonomy at all times.

    Pay: Competitive salary, regular salary increases, potential for performance awards
