- High School graduate or equivalent
- Post-secondary training in human services or related field/experience working with people with disabilities may be substituted
- Two (2) years of work experience
- Excellent communication skills (verbal and written)
- Provide leadership, supervision, training and support to Case Coordinators and Training Specialists in designated service location
- Provide case management services and maintain a caseload of Long-Term Support Clients supporting them in a variety of job sites
- Provide direct skill development/job coaching support for individuals at their community job sites
- Understand various disabilities and other barriers to employment
- Responsible for creating/maintaining schedule of staff supports for assigned clients
- Observe and evaluate work performance and develop goals for individuals providing feedback and observations to Employment Services Staff
- Provide problem solving and job-related counseling
- Provide communication to/with teams (funding sources, employers, families/caregivers, internal staff)
- Ability to document and case note timely reports weekly and monthly within required timelines
- Assist with tracking data pertaining to end of month/year reports
- Possess awareness of realistic work expectations regarding quality of work and appropriate behavior
- Ability to effectively communicate in order to act as a liaison between employers and employment services staff as well as individuals care team (funding sources, employers, families/caregivers)
