    Job Description

    Good Shepherd Hospice is looking for a caring LPN to support patients and families in Duncan and nearby areas. As an LPN at the hospice, you will work alongside the RN to assess, plan, and provide care in line with the Interdisciplinary Plan of Care, prioritizing the well-being and comfort of patients and families.

    Key Details:

    • Full-time position
    • Prior experience in hospice care is necessary
    • On-Call duties required

    Key Responsibilities:

    • Receive patient assignments from the Patient Care Manager
    • Communicate with RN Case Manager, hospice staff, and other agencies to ensure optimal care
    • Develop communication and bereavement skills
    • Maintain regular communication with the attending physician
    • Update patient records accurately and maintain clear documentation
    • Provide support and education to patients and caregivers
    • Inform RN Case Manager of any issues
    • Attend team meetings and patient conferences as needed
    • Offer support during end-of-life care and provide 24/7 coverage
    • Learn about hospice philosophy and pain management

    Skills and Qualifications:

    • Prior experience in hospice care
    • High ethical standards and ability to maintain confidentiality
    • Team player with a commitment to following care plans
    • Pass background checks successfully
    • Graduate of an accredited LPN program
    • Active state license as a Licensed Practical Nurse
    • Valid driver's license
