- Master's in Social Work required from a school accredited by the Council on Social Work Education.
- One (1) to two (2) years of clinical social work experience required. Three (3) plus years of clinical social work preferred. Experience working with interdisciplinary team preferred.

Social Worker MSW
PRN Position - Shifts Options - Monday - Friday, 8am - 5pm - Weekends as Available
Overview:
The Social Worker provides full range of hospice social work service and bereavement support to patient and families in the care center, home and institutional settings.
They work with interdisciplinary team to provide directly, or through coordination of team members, appropriate assessment of and support to patients and families.
Also, this position serves as liaison with other community resources; assists staff, patient's physician and volunteers in understanding significant social and emotional factors affecting a patient and family during terminal illness.
The Social Worker provides assessment for normal and complicated grief and provides bereavement counseling as appropriate. The Satterwhite Campus for Care and Compassion houses Haven's Palatka Offices and the Roberts Care Center which is a state-of-the-art hospice care center expanded in 2013 to 18 private patient rooms.
The care center was designed to include all of the comforts of home in a home-like environment and provides for complex medical and palliative care needs.
First opened in 2003, the Roberts Hospice Care Center serves Putnam, Clay and Flagler, Volusia and St. Johns counties. The care center is located at 6400 St. Johns Avenue, Palatka.Environment:
