    Description

    Clinical Liaison

    We are seeking a dynamic and experienced Clinical Liaison to join our team and serve as the primary point of contact between multiple physician practices, specialty clinical services, patients, and providers. The Clinical Liaison will act as a quarterback and clinical advocate, coordinating and facilitating the seamless delivery of healthcare services while ensuring patient and provider needs are met effectively. The ideal candidate will possess excellent communication skills, clinical expertise, and a strong ability to build and maintain relationships across various healthcare stakeholders.

    Responsibilities:

    • Serve as the primary liaison between multiple primary care practices, specialty clinical services, and population health analyst helping to enhance care and make clinical meaning of the data.
    • Collaborate closely with physicians, nurses, and other healthcare professionals to ensure coordinated care delivery for patients.
    • Act as a clinical advocate, advocating for the needs and preferences of patients while adhering to clinical best practices and guidelines.
    • Coordinate patient referrals and transfers between different healthcare facilities and specialty services as needed.
    • Ability to work in multiple EHRs to facilitate patient care, appointment scheduling, bidirectional patient care information.
    • Critical eye for chart reviews and data analysis, helping to identify clinical opportunities for improvement at the population and patient level.
    • Utilize risk billing and coding knowledge to accurately assess patient risk profiles
    • Conduct thorough assessments of patient needs, ensuring timely access to appropriate clinical services and resources.
    • Participate in quality improvement initiatives, in collaboration with internal and external stakeholders, to contribute to the development of clinical protocols and guidelines.
    • Facilitate communication and collaboration among interdisciplinary healthcare teams to optimize patient outcomes.
    • Maintain accurate and up-to-date documentation of patient interactions, referrals, and care plans.
    • Stay informed about advancements and changes in healthcare regulations, policies, and practices relevant to the role.

    Qualifications:

    • Advanced Practice Provider Clinical licensure (e.g.,nurse practitioner, physician assistant) in Georgia, required
    • Master's degree in nursing, public health, healthcare administration, or a related field preferred
    • Previous Medicare Advantage experience as a care provider or support preferred
    • Minimum of 5 years of experience in population health management, care coordination, or a related healthcare role.
    • Strong understanding of HCC codes, accurate code capture, and risk risk adjustment models (e.g., HCC, ACG, DxCG).
    • Excellent communication and interpersonal skills, with the ability to engage and collaborate with providers, staff, and other stakeholders.
    • Demonstrated ability to work independently and collaboratively in a fast-paced, dynamic environment.
    • Strong analytical and problem-solving skills, with attention to detail and the ability to synthesize complex information.
    • Knowledge of healthcare regulations, policies, and quality metrics related to population health management and value-based care.
    • Willingness to travel locally as needed to work with multiple provider groups and healthcare facilities.
    • Requires > 50% travel in assigned geography

    Compensation: $114k-$141k/annual salary (national average, premium markets may vary)
