    The Pharmacy Technician Med Reconciliation position is responsible for accurately collecting and entering current patient medications into the hospital's information system for utilization by the physicians responsible for admitting and discharging the patient from the hospital. Is responsible for relaying any issues that cannot be addressed by the Specialist to the pharmacist and/or physician. Performs limited medication management and distribution.

    Responsibilities
    • Performs all responsibilities/duties required by the pharmacy as defined by the scope of service to ensure that the unique nature of the client is addressed.
    • Exhibits solid knowledge of medications, strengths available, indications. Excellent ability to use drug information database products to obtain medication information needed to perform assigned functions.
    • Conducts patient or caregiver interviews in order to obtain best possible medication histories for patients admitted to the hospital, including prescription, over the counter, herbal, and nutraceutical medications.
    • Communicate with external pharmacies, physicians or other third-party software regarding any patient medications, refill dates, instructions for use, etc. and confirm current medication history as described by patient where appropriate for clarification, completion of medication history or to resolve any discrepancies.
    • Coordinate with nursing, case managers, and medical staff to identify patients who need a medication history interview.
    • Communicate with pharmacists on identified problems and issues discovered during review and interview, including but not limited to formulary substitutions.
    • Accurately transcribe medication histories into electronic medication health records for review.
    • Develop draft admission medication reconciliation list for physician review and processing.
    • Sets priorities in correct order in relation to the needs and circumstances at the time with reference to the patients, co-workers, physicians, and the nursing unit, etc.
    • Responsible for conducting in-service education lectures when assigned for medical staff, nurses, pharmacists, students, and other health care personnel regarding drugs, procedures, and policies.
    • Represents pharmacy services in interdepartmental meetings regarding medication reconciliation processes and procedures development and refinement.
    • Represents pharmacy services as part of the integrated interdisciplinary patient care team with medication reconciliation.
    • Exhibits excellent verbal and telephone communication skills with the ability to quickly create and maintain interpersonal trust with patients, family members, outside professionals, to the extent needed to perform job functions.
    • Maintains current knowledge/certification/licensure. Pursues professional growth and development.
    Qualifications

    Minimum Education
    • High School Diploma or Equivalent Required
    Licenses and Certifications
    • Registered Pharmacy Technician State if Florida Upon Hire Required
    Required Skills, Knowledge and Abilities
    • Basic computer skills including MS Word and the Internet
    • Good oral and written communication skills.
    • Understanding of the medication reconciliation process.
    • Ability to document medication history information using available resources.
