    The purpose of the Nurse Navigator position is to support the interdisciplinary team in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payors.

    The role specializes in understanding insurance site-of-care requirements, high-cost biologic infusion therapy, and autoimmune conditions.

    Demonstrates a high level of understanding of biologic drug treatments and related disease states (primarily autoimmune including RA, AS, UC, Crohn's, and certain forms of cancer treated with biologic therapy)
    Coordinates care for all patients on home infusion census within interdisciplinary team (providers, pharmacists, nurses, pharmacy techs, financial navigators, billers)
    Ensuring timely access to care
    Ensuring appropriate site of care (home vs clinic)
    Identifying patients with high risk for non-compliance or disease progression
    Tracks patients referred to home infusions ensuring tolerance of medication prior to transferring site of care
    Performs initial phone assessments prior to initial home infusion to ensure appropriateness of site of care and supporting patient through care transition
    Assists with program applications, as needed
    Identifying appropriate site of care for treatment
    Facilitation of the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement
    Leads interdisciplinary care coordination meetings
    Collaborates with home infusion nurses to identify patients at high risk for disease progression
    Coordinates communication with ordering providers and other members of patient's care team
    Coordinates/facilitates patient care progression throughout the continuum.
    Reviews patient charts to identify appropriate site of care and insurance payments

    Collaborates with nursing team to develop department guidelines and standards of care Works collaboratively and maintains active communication with physicians, nursing and other members of the multi-disciplinary care team to effect timely, appropriate patient management.

    Works collaboratively with insurance companies to coordinate appropriate site of care decisions and prior authorizations for infusions
    Seeks consultation from appropriate disciplines/ departments as required to expedite care.
    Collaborates with the physician and all members of the multidisciplinary team to facilitate care for home infusion nurse therapy census; monitors the patient's progress, intervening as necessary and appropriate to ensure that the plan of care and services provided are patient focused, high quality, efficient, and cost effective; Modifications of plan of care, as necessary, to meet the ongoing needs of the patient;
    Communication to third party payors and other relevant information to the care team;
    Includes obtaining physician co-sign on care plans every 30 days for patients on home infusion census.

    Collaborates with medical staff, nursing staff, and ancillary staff to eliminate barriers to efficient delivery of care in the appropriate setting.

    Ensures that all elements critical to the plan of care have been communicated to the patient/family and member of the healthcare team and are documented as necessary to assure continuity of care.

    Actively participates in clinical performance improvement activities.

    Assists in the collection and reporting of resource and financial indicators including costs of care, and management of chronic conditions.

    Uses data to drive decisions and plan/implement performance improvement strategies for home infusion patients, including fiscal, clinical and patient satisfaction data.

    Collects, analyzes and addresses variance from the plan of care/care path with physician and/or members of the healthcare team.

    Collects delay and other data for specific performance and/or outcome indicators as determined by Nurse Supervisor or Pharmacy Manager
    # Ensures safe care to patients adhering to policies, procedures, and standards, within budgetary specifications, including time management, supply management, productivity, and accuracy of practice.
    # serves as a preceptor, mentor, and resource to less experienced staff.
    # Demonstrate Standards of Behavior and adhere to the Code of Conduct in all aspects of job performance at all times.
    Being fully vaccinated against COVID-19 is an essential requirement of working at Confluence Health. Religious and medical exemptions will be considered upon request.
    Strong analytical, data management and PC skills.
    the ability to organize and manage time constraints, to independently manage tasks including basic computer skills and EMR.
    O = Occasional, represents 1 to 25% or up to 30 minutes in a 2 hour workday.
    F = Frequent, represents 26 to 50% or up to 1 hour of a 2 hour workday.
    C = Continuous, represents 51% to 100% or up to 2 hours of a 2 hour workday.
    Job requires both working closely with others and working alone.

    Work is performed in a hybrid of in-office and at home, both of which consist of sitting at a desk for long periods of time.
